Online Referral Form

Capture5.JPG

Who Can Make a Referral?

We have been listening to the local community, and have now decided to accept self-referrals. This means you are now able to deal with us directly. Just to ensure our support reaches those in genuine need, we may sometimes request proof of need. This may require you to supply proof of income. By submitting a referral form you are agreeing to this.

Front line professionals such as Social Workers, Health Visitors, GP's, Schools, Food Banks, Safeguarding Team and Local Authority etc are still able to refer families in need by completing the form below.

What Support Can We Provide?

We are able to provide support to children from birth to 11 years by offering the following: Clothing, nappies, wipes, formula, baby food, buggies, cots, toys and books.

All of our products are either new or pre-loved and in good condition.

We will always try and source all items requested, however we are not always able to guarantee it.

How does it work?

If you or your supported family identifies as requiring our support, please complete this online referral form (with the consent of the family) with as much information as possible.

Once we have received the form we will contact the family directly (normally within 48 hours) and confirm their needs. We will then arrange a time and place for the family to collect their items.

If the person making the referral on behalf of the family would like to collect, please let us know.

How Often Can I Make Referrals?

There must be a minimum of 3 months in-between referrals for the same family.

If you need any help completing this form, please call us on: 07379575792
